The objective of this chapter is to analyse the current literature that addresses the use of the patency file in endodontic therapy. Histological sections of the apical thirds of teeth that were appropriately cleaned with the use of this procedure and then examined usually show remnants of organic tissue and biofilm. Detractors of the use of the patency file have concerns with the increased extrusion of debris and irrigants through the foramen, higher incidence of postoperative pain, lack of proper studies showing its ability to improve cleaning of the complex anatomy of the root canal system in the apical third, and limited information on the influence of the use of the patency file on the prognosis of the root canal treatment. This procedure is even cited as an important step in achieving clinical success. Using a small K file inserted passively and intentionally by 1 mm through the foramen is advocated by some investigators and clinicians as an important procedure designed to help the cleaning and shaping process, to aid in delivering irrigating solutions to “hard to reach areas” of the root canal system, as well as to the apical third.
